Zuri Health launched its Continuous Care Program (CCP) to offer support to persons with Chronic illnesses as part of its mission to democratise healthcare in Africa.

The Launch Event took place on December 21, 2022, at the New Lavington Mall Pharmaplus Pharmacy brunch.

Zuri Health has partnered with Pharmaplus Pharmacies Limited, a trusted and respected name in the pharmacy industry.

This collaboration aims to transform chronic care patients' lives by offering them a wider range of healthcare services, including medication management and access to the Continuous Care Program.

"People suffering from chronic illness have complex health needs that current health-care delivery systems do not address or provide in a seamless manner," they said.

The care program is people-centred and seeks to cater to and provide ongoing support to an individual's health needs and desired medical outcomes.
